If you are age between 40and 74 with No pre-existing medical conditions (e.g. heart conditions, diabetes, Cholesterol, Blood pressure medication) then we would like to invite you for a FREE NHS health check.
When you visit the practice for the first time either to see a doctor or any other health professional, the reception staff will ask you to do your blood pressure, height and weight. This information will go onto your records.
An NHS Health Check will offer guidance on how to prevent developing a chronic long-term condition such as:
- heart disease
- stroke
- diabetes
- dementia
- kidney disease
In order to book the health check please phone the surgery to make an appointment with any of our nurses or healthcare assistants 2 weeks after you had a your blood test done as you will need to have blood test first before the initial appointment
